BHA Landlord Forms

• Request for Rent Increase: available both in an online FORM* or printable PDF – for landlords who wish to request a rent increase on a participating tenant receiving Section 8 rental assistance. *If you submit more than 5 in one day, please email landlord@bremertonhousing.org with a list of the tenant names and unit numbers so we can ensure all are accounted for.
  • Rent Survey: how we determine Rent Reasonableness. Available both in an online FORM or printable PDF. Included in the Request for Rent Increase form.
• Direct Deposit (PDF) and W-9 (PDF): for landlords who wish to sign up for direct deposit payments or change their existing banking information.
• Landlord/Owner Change (PDF): for landlords who have a change of ownership, property management, direct deposit, or contact information.

Inspections

• HQS Pre-Inspection Checklist: a 1-page checklist that landlords can use to give a better understanding of what a Housing Inspector looks for during inspections.
• HUD HQS Initial Inspection Flowchart: visual flowchart of the process of the inspection before move in. Note that where it says “HUD” in the first step, it means “BHA”, and “PHA” is the same as BHA.
• WA State Department of Commerce’s Move-In/Move-Out Condition Report: a sample move-in/move-out form offered by the WA State Department of Commerce.
